Exploration of the family context


Problem:

In the YMH Boston Vignette 5, what I appreciated most was the clinician's calm pacing with Tony. At first he minimizes feeling sad and does not quite understand the mood questions, but the practitioner stays with him long enough for the real story to come out: the breakup, the drop in grades, the loss of energy for basketball, and finally the comment that he does not want to be alive. Where I think the interview could improve is around structure. There is very little exploration of family context, home stressors, or supports, and the quick check on alcohol use is left hanging instead of being folded into a broader risk picture. By the end of the clip my main concern is safety. Tony has clear functional decline, strong anger, physical anxiety symptoms and emerging suicidal thinking. My next question would be very direct: when he says he does not want to be alive, has he ever thought about specific ways he might hurt himself. That one question immediately guides the rest of the visit, including the level of observation, the need for urgent family involvement, and whether he can safely return home. Do not answer yet.
