In following the recommended "lookback" procedure, it was determined that the platelet concentrate had not been used, the FFP was still frozen, and the RBC had been transfused to a 48-year-old woman undergoing hysterectomy for dysfunctional uterine bleeding associated with multiple, large uterine tumors.
