What is the difference between an epidemic and a pandemic?

There have been many disease outbreaks around the world, some localized and some widespread. When is a disease outbreak a concern? What factors result in the spread of these diseases?

There are disease outbreaks that occurred a long time ago, such as the black plague, which claimed many lives in Europe. There are diseases that have been around for a while but due to vaccination are currently being held in check in most places, such as diptheria and tuberculosis. There are other diseases that scientists are still working on developing a vaccine for such as Malaria and HIV. Some recent diseases, such as Ebola, have very localized outbreaks. More recent diseases to have large outbreaks include H1N1 and the Zika Virus. How are these diseases spread? Why do some of these diseases remain localized while others become more widespread?
