Problem: 11-years-old female present for an initial psychiatric evaluation. Patient reports a past diagnoses of Bipolar II, ADHD, GAD. Patient has been on and off psychomed for years. Last medication was Wellbutrin which she thought was effective. Patient relocated from Dallas back home to Kansas to live with parents in January 2025. Patient was seeing a therapist prior to moving back to Kansas. Since the transition back home, patient stopped seeing a therapist and stopped taking medication because she was feeling pretty good. Since November, patient reports feeling more depressed and having self-harm thoughts again. Patient is currently seeking renewal of medications. Full mental health comprehensive assessment performed.
