Problem
You are the CHRO of Big City Medical Center. The CNO of BCMC comes to meet with you to discuss a no-fault medication error plan. Nursing literature shows that a no-fault system causes greater reporting of errors. With a better understanding of errors, remedial actions can be taken to improve quality in medication administration. You are responsible for the medical center's disciplinary system. You have routinely disciplined RNs and other clinical staff for clinical errors including medication errors. What do you say to the CNO?
