Description of the client---describe

Identifying information (e.g., name, gender, date of birth, relationship status, cultural background).  General description of the client's appearance, mood, and level of cooperation during the interview.  Note any observations about their memory, thinking/speech patterns, signs of anxiety, depression, or other mental health issues.

Presenting problems---most urgent problem, the problems that bring your clients to you

The reason why the client came to you.  The client's description of their problem, how long they have been experiencing the issue and what they hope to gain from seeking your services.  Description of what the client has done in the past.

Biological assessment (including medical history)---describe

(APA Definition: Biological factors include any chemical, physical, neurological or genetic conditions association with psychological disturbances.  Consider: genes, age, infections, immune response, environmental toxins, physical traumas, diet appetite, hormones, sleep/exercise habits)

Psychological assessment ---describe

(APA Definition: Factors that contribute to the development of personality, the maintenance of health and well-being, and the etiology of mental and behavioral disorder. A few examples of psychological factors are the nature of significant childhood and adult relationships, the experience of ease or stress in social environments (e.g., school, work), and the experience of trauma.  Consider: personality, psychological traumas, stress, coping skills, suicidal ideation, reaction to illness)

Social assessment (including environment)---describe

(APA Definition: Social conditions that affect human behavior. Examples of such factors are socioeconomic and educational level, environmental circumstances (e.g., crowding), and the customs and mores of an individual's social group.  Consider family relationships, social support, relationship status, cultural influences, spiritual or religious backgrounds, hobbies and recreational activities, living situations, military service, financial status, sexual history, legal history, educational background, employment status and work history)

Observations: Interview skills and empathy of the social workers in the video - What did the interviewer do well? Verbal and non verbal from the clients, what do those verbal and non verbal clues show you? How do you interpret those clues?  If you were the social workers, what challenges did you observe and what would you do differently?

Observations about self: What did you notice about your own reactions and emotions to the client, interviewer, or any part of the interview experience?  What did you learn about yourself?
