a. Identify who is part of your Service community.
b. Which service community members have participated in the self-assessment?
c. How have you encouraged and supported meaningful participation from all of your service community members?
d. How have you included children's voices, feedback and opinions during this self-assessment process?
e. How did you share the self-assessment data with your colleagues in the service?
f. Provide a summary of discussions you have had with Supervisor and/or colleagues, including date of discussion and name.
