For this assignment, first download and print out the PDF version of the questionnaire so that you can write down your responses. (If you'd prefer, you can also use the editable Word version.) Then read through questions 4-31 and give a simple yes or no answer to each. These questions explore experiences associated with the development of consciousness, and you may be surprised by how many "yes" responses you have. Usually a few of these questions will stimulate your memories of specific experiences and/or benefits, and in those cases, please choose at least two and write a short elaboration of each to submit as your forum assignment (150-300 words total). You may also submit your full questionnaire if you'd like, but you're not required to do so. Once you've posted your response, please comment on at least two submissions from your classmates.
