How do you approach recognizing ethical issues that arise

Problem

A person who wishes to act ethically must (i) recognize ethical issues when he or she runs across them (moral awareness); (ii) have the ability to reach a defensible resolution of the question as to what is the right thing to do in that setting (moral decision-making); (iii) desire to do the right thing (moral intent); and finally, (iv) be able to act on that intent (moral action).

Think about your own experience with moral awareness in the workplace. How do you approach recognizing ethical issues that arise at work? Who do you think should be held responsible for making ethical decisions at work-you or your boss? Why? What are your strengths and weaknesses when it comes to doing the right thing at work?
