Write a follow up psychiatric note, chief complaint, current Meds, HPI, formulation, recommendation treatment plan for a 33 years old female with diagnosis of Attention-deficit/hyperactivity disorder, Predominantly hyperactive/impulsive presentation, Generalized anxiety disorder, Current Meds: dextroamphetamine-amphetamine ER 20 mg 24hr capsule,extend release; hydrOXYzine HCl 25 mg tablet PRN daily. HPI: she Said she just got a job and very happy about it. Working as an assistance manager in the Friendship house at Mt, Vernom.. she gets up about 6am and takes her Adderall at that time, but by 2pm she starts struggling with her focusing, concentration and getting task done. Patient verbalized wishing to switch to immediate release Adderall two times a day. She said she also quit the Venlafaxine and it was initially rough for her but she has settled in finally and feels good . She said she has been able to also approach her dad and how he treated her as a child. She said her parents divorced since when she was 5years old but whenever they talk, all he talks about bitter divorce with his mom and this has really affected her.. She said she had to take Hydroxyzine.PRN as needed when approaching her teenage kids. . She said last two weeks, she had a lung infection and struggled to sleep, but she feels better now and her sleep has been good. She denies SI and HI. Need Assignment Help?
