Problem: Develop a short survey and short interview questions about health status and health care, with only 20 items total. Make sure to design it so you are collecting basic background data on a patient (i.e. age, sex, race, educational level, marital status, employment status), as well as open ended questions to glean more specific information about their health status and history.
