A substance use disorder counselor must understand tolerance, synergistic effects, and withdrawal to accurately assess clients, recognize medical risks, and develop effective treatment plans. Tolerance occurs when a person needs increasing amounts of a substance to achieve the same effect, which may increase the risk of overdose and dependence. Withdrawal symptoms can range from mild anxiety to severe medical emergencies such as seizures or delirium tremens, especially with alcohol or benzodiazepines. Synergistic effects occur when substances are combined, producing intensified effects that can be dangerous or fatal (Capuzzi & Stauffer, 2024). For example, combining alcohol with opioids may severely depress respiration and increase the likelihood of overdose. Without this knowledge, counselors may misinterpret symptoms, fail to identify medical emergencies, or provide inappropriate interventions. Substance use disorders can also mimic or co-occur with neurological, medical, and psychological disorders. According to the Diagnostic and Statistical Manual of Mental Disorders (5th ed., text rev.; DSM-5-TR), symptoms related to substance use may resemble depression, anxiety, bipolar disorder, psychosis, or cognitive disorders. For instance, stimulant intoxication may appear similar to mania, while alcohol withdrawal can resemble anxiety disorders. Counselors must carefully evaluate whether symptoms are substance-induced, independent mental health conditions, or both to ensure accurate diagnosis and treatment (American Psychiatric Association, 2022).
