Problem: James Gordon is a fifty year old male. He is being seen in the emergency room for shortness of breath. This came on rather suddenly. He is alert and oriented but somewhat anxious.
His pulse is 120, BP 160/90 mmHg, respirations 25, shallow and slightly labored. Chest expansion is decreased bilaterally; slightly decreased vocal fremitus throughout; dull percussion note throughout; breath sounds are decreased bilaterally with crackles on inspiration. Patient's skin is warm and moist; lips and nail beds are slightly cyanotic.
