A. What is the public health problem that is being addressed? Who is the target audience?
B. What are the malleable risk and protective factors? In other words, what is the program trying to change?
C. What is the program, policy or practice? In other words, what is being done to bring about change?
D. How is it being disseminated or diffused? In other words, how is the target population being reached?
E. Include a link to the website or copy of the article where you found the program, policy or practice.
