Problem: Describe how this post was a good outline plan that offers understanding of hospitals readmission rates, and what useful information could I respond with: Supervisor, I understand that hospital staff would like a better understanding of the readmission rates. This would be beneficial to both staff members and patients, as they would need to know this as well. I have completed a plan outline that can be passed on in the next staff meeting.
a. The goals of the analysis need to be correctly defined: reducing admission rates, avoiding penalties from Medicare and improving patient outcomes.
b. Determine what key performance indicators are related to the readmission rates. An example could be the reasons for readmission or demographic factors.
c. At this stage, data needs to be collected to develop a plan. This data should include discharge summaries, medical records, patient demographics, and any follow-up appointments that are needed.
d. Identify the stakeholders that would also need access to the plan and information such as nurses, quality improvement teams, hospital admin and physicians.
