Assignment:
For this assignment you will complete a Hazard Analysis Report. A site analysis and the report that follows is a key element in any study of occupational health.
A hazard analysis report is written once a site analysis has been conducted. Your report will summarize the findings in the analysis, the demographics of the company, and make recommendations based upon findings. Compose a report that summarizes the findings from your completion of the hazard analysis worksheet, discusses the demographics of the company, and finally makes recommendations based upon your findings This means that the audience for this paper may not be a professional in the field of occupational health.
The Hazard Analysis Report should include the following:
? Introduction
? Background of company or site (much of this information will be provided in the information packets)
? Worker demographics (age and sex)
? Is there a safety committee?
? Are there any OSHA citations?
? Examine records and incident rates
? Site Analysis
? Summary of datasheet information.
? Calculations for certain items (eg. calculations for lifting analysis or Job Content Questionnaire)
? Include your completed datasheet as an appendix.
? Identified hazards from site analysis
? List all hazards identified from the completed site analysis.
? Prioritize the hazards using your analysis and the company records.
? Recommendations
? Identify and discuss potential interventions that could be implemented for the most important hazards identified.
? What types of controls could be implemented to avoid the most important identified hazards. These controls could be engineering, administrative, and/or PPE.
? Bibliography
? You need to support your claims throughout with reputable resources.
? All citations must be formatted using the most current version of AMA style.
Requirements for Submission: Submit a 10 page word document (not including bibliography and appendices) double spaced- 11pt Times -New Roman font with -1 inch margins. All citations and other formatting AMA style.
