You are a nurse.  You suspect one of the physicians that you work closely under has a substance abuse problem that is affecting his work.  You think this because he has made a few more mistakes lately and because you believe you have detected booze on his breath occasionally.  What do you do?

A patient you are treating is very sick.  Due to treatment for another illness, she is immunosuppressed.  She is supposed to take antibiotics prophylactically (to prevent infection).  She now has lung infection (pneumonia) which could easily be fatal if not treated.  She hates pills and often refuses to take her medication which is why she developed this lung infection.

Today you are told that she is again refusing her medication.  Each time she does this, it results in another long conversation with the patient, which you are beginning to dread.  To save yourself the headache you could capitulate to the patient's wishes without arguing and have the nurse just skip the dose, but the patient will almost certainly get worse today and would die if this is done repeatedly.
