Please note that after reviewing and signing off on IRs, I realized that several IRs have not been completed. Please ensure that you enter all Incident Reports by the end of your shift to ensure that all parties are notified and that our workflow remains efficient. Outside of our internal notifications, every youth team needs to be notified when incidents occur, and the notification should happen within 24 hours. Need Assignment Help?

Undocumented Incidents:

Physical altercation between Carmella and Cordelia, Was nursing notified? Were youth seen by nursing?  There is no documentation or nursing assessments in carelogic. Please follow incident protocol posted in the office as a guide. 

Sofia was involved in an incident on 08/28 where she was destroying her roommate's property, throwing items down the stairs in the hallways, etc. This needs to be documented in the IR database.

Sofia struck Carmella with a purse and attempted to fight Carmella this needs to be documented.

Today's restraint with Sofia was not documented. Immediately after the incident, I took her from the house, ensured nursing was notified, and they recommended that CareLogic shift notes, incident reports, the incident protocol posted in the  facility, and psychoeducationshe be seen in the ER. Again, no IR or email notification was sent out to let internal team members know what happened. Moving forward, I request that if an incident involving Sophia occurs, I be notified via text and/or email during that shift.

Finally, individual shift notes should reflect behaviors exhibited during the shift, good or not so good. If an incident occurs during the shift, it should be documented in the child's CareLogic report, as this is what the Office of Licensing reviews.

We are approaching our Office of Licensing Inspection, so I want to ensure our charting meets expectations. To better support everyone within the home, Justice Bozarth will be facilitating two training sessions next week that will focus on charting, CareLogic shift notes, incident reports, and psychoeducation notes. I will send the dates and times; everyone will need to make at least one of these trainings. Offering support and training will assist your development within the job as well as help us ensure we are meeting OOL guidelines.
