Possible angina pectoris or atherosclerosis


Problem:

The following consult note was documented on hospital inpatient Don Wentworth. Patient seen today at the request of Dr. Stone. Patient complains of chest pain, which is present at the moment and rates a 5 on a scale of 1 to 10 (with 10 being the worst). The patient has a past medical history of hypertension treated with medication for 10 years. Recently diagnosed with diabetes, controlled with diet and oral medications. Current blood glucose was 378. EKG has slight elevations. Plan is to admit patient, repeat chest X-ray, EKG, and cardiac enzymes.

IMPRESSION: Possible angina pectoris or atherosclerosis. Will provide further workup for definitive diagnosis in the hospital.
