Question: Mr. J, 54 years old Male, limited English speaking background originally from Nepal, staying temporarily in Australia since 1 year. He has a chronic medical illness of high cholesterol and diabetes type 2 since 6 years and hypertension since 10 years. Mr. J is currently on medication Gemfibtozil 600 mg daily to control cholesterol, Metformin (APX) 500 mg twice a day for diabetes and Telmisartan 40 mg once daily to control his blood pressure. This case study will be more focused on hypertension. He had a family history of hypertension and diabetes. Currently, he is physical less active , not working and not having appropriate diet plan. Reflecting at his diagnosis time Mr. J was experiencing severe headaches, dizziness, increase in heart rate and anxiety. He had a habit of seeing a doctor when symptoms arose or worsened back in overseas. After coming to Australia, he is visiting a GP every month, however not monitoring his blood pressure regularly, regardless of his medical condition. Currently facing more challenges with his mobility and stoke risk due to his being overweight, and high cholesterol. GP has suggested visiting a dietitian for diabeteic and low cholesterol diet plan and weight loss.
