Include the following:

• Provide an historical overview of health care reimbursement models including fee for service, Preferred Provider Organizations (PPOs), managed care, and Health Maintenance Organization (HMO) configurations.

• Assess how each of these models has contributed to the cost of health care delivery. Include your comments with regard to deductibles, co-payments, and exclusions for pre-existing conditions.

• Assess how government entitlement programs such as Medicare and Medicaid have enabled or disabled the delivery of health care in the United States.

• Recommend how the economics and financing of health care should be structured and why.

The paper must be formatted as follows:
