They was informed that a CHHA referral was sent to Extend Homecare for a nurse, home health aid and PT/OT. The writer explained that before services are provided Extended Homecare will send a nurse to complete an evaluation to determine the amount of services he will receive. They will reach out to him to provide the start of care date. The writer also took the opportunity to schedule a New York State Independent Assessor (NYIA) for the enrollment into a managed long term care insurance. It is a two part assessment, the first assessment is schedule for September 8, 2025 12:30 PM with nurse Marsha Lee (ID#: 573358) and the 2nd assessment is scheduled for September 9, 2025 AT 3:00 PM with Nurse practitioner Alec (ID #: 476207). Mrs. Lowery also hired a private aid Monday to Sunday for 8 hours each day. Prior to leaving she also received wound care training. The transfer discharge notice was reviewed and completed.
