Purpose(s):

1. To better understand what essential components are included in a community assessment.

2. To be able to identify the resources needed to secure the necessary assessment data.

3. To evaluate the strengths and weaknesses of the community dimensions

4. To prioritize the problematic areas and formulate an appropriate nursing diagnosis.

5. To utilize public health and or nursing research to support your nursing interventions.

6. To strengthen critical thinking and writing skills.

Assignment: Your goal is to develop a nursing diagnosis within a community assessment, meaning that whatever nursing plan you devise, you must contextualize it to work within community factors.

Section I: (5 pages) Offer the background on the community itself, highlighting and detailing the most relevant factors to your diagnosis, as well as offering a background on the patient. This section should include a detailed description of all 6 dimensions of a community assessment (see separate attachment)

Section II: (5 pages) Based upon your assessment in section 1, develop a nursing diagnosis. Justify your diagnosis within the community context by explaining why you are making the best choice(s) for the patient. Describe at least three evidence-based nursing interventions. Discuss why you chose those particular interventions.

Requirements: APA format. Please attach a copy of the articles you used to support your interventions. You must use at least
3 (peer reviewed) sources.

Tips for Success: This assignment will be assessed using the USJ Target rubric. Therefore, in section I of your piece, I want to see you pay attention to the need for clear organization and detailed summary on the patient and the community context. In Section II, pay attention to the need for A.L.I.V.E. Evaluative Work and Complexity, meaning that I want to see how well you can weigh various factors, such as patient needs, against other factors, such as what is realistic relative to the community situation, and vice versa. Merely offering a list of things you could do will result in low marks. I am not interested in learning about possible interventions, I am interested in seeing that you can justify your choice of an intervention(s) over other possibilities. Also remember that you are allowed to engage source material and evaluate discrepancies between different sources and how this impacts your ability to make good decisions

Below is the patient you are using to write the intervention for. Her primary diagnosis is Type II Diabetes Mellitus

I will want you to use diabetes with nursing diagnosis: Ineffective Health Management Below is the description of the patient

1. Describe the Patient:

a. Patient was a 65-year-old woman.

b. Patient Medical Diagnoses: Primary diagnosis was Type II Diabetes Mellitus. Additional diagnoses include Hypertension and Hypothyroidism.

c. Patient’s Medications: Patient was on Glipizide 10mg tablet to treat diabetes mellitus, metformin 850mg to treat diabetes mellitus, amlodipine (Calcium channel blocker) 10 mg tablet to treat high blood pressure, hydrochlorothiazide (water pill) 25 mg tablet to treat high blood pressure, Labetalol (beta blocker) 40 mg tablet to treat hypertension. Levothyroxine 112 mcg tablet to treat hypothyroidism.

d. Patient’s Family Structure (children, who’s available and caring for them?): This patient live with her husband of forty-five year old. She has four grown children who live on their own and eight grandchildren.

e. What Community do they live in (be more specific than “Hartford”): The patient lives in an apartment building one and a half miles from downtown Hartford. The building is situated in a busy neighborhood on Main Street closer to convenient stores. This part of Hartford has an area code 06120 and its part of Hartford County.
