Is the waterfall model really still being used anywhere

Problem

• [#Waterfall] Fowler (and Farley) seem to think that a lot of people are claiming to use Agile Development process, but they are still woking in a Waterfall mindset. Is the Waterfall model really still being used anywhere? Either explicitly or implicitly? Can you find any examples of this on the web (provide a link) or do you have any direct experience with it?

• [#Iterative] Both Farley and Fowler seem to think that as long as your process is iterative, you are on the right track. Do you agree? Or do you think there are important difference between different iterative processes like the Spiral Model, Extreme Programming, the Unified Process Model, and DevOps? Have you had direct experience with more than one process model?
