Installing pipe sections onto a sewer line

Question: Your workers are installing pipe sections onto a sewer line and using a trench box for cave-in protection. A worker is in the trench box and using a tag line to help control and guide the pipe while it is lowered with an excavator into the trench for placement and connection to the already installed end of the existing line. It is not permissible to be underneath loads handled by lifting or digging equipment. In an LOI dated 12/19/2007, due to the compact and limited work space in the trench, and the hazard of a falling load pining a worker between the pipe and trench box, it is deemed a GDC violation. What is a reasonable solution? Limit workers in the trench to 1 person with tagline while hoisting Exiting the trench, guiding with a tagline, and re-entry when the pipe section is within inches of the ground Using two trench boxes so that the work area where the pipe is being lowered, can be cleared to the second box, with the tagline controller using the structural members of the front box for protection, until the pipe is within a few inches of the ground B or C
