Problem
• Identify key characteristics of community-based action planning and discuss how this approach could enhance community health initiatives. Justify the benefits of the Healthy Cities and Communities Model strategies.
• Discuss what are Leading Health Indicators (LHI) and how they serve to monitor and prevent major health concerns such as mental health, maternal and infant health, nutrition, and substance abuse.
