The Community Intervention Action Plan is introduced in the log template item #3. Use the template provided in the log document to organize and write the plan. The completed plan is to be submitted separately from the weekly log using this assignment link.
During each CLE experience, you will interact with a population in need. You will observe interesting situations that occur during your volunteer hours. Review these situations through a "Nurse's eye" and apply the Social Determinants of Health. Identify a health need associated with this population.
As a health care worker, it is a goal to optimize healthcare delivery and improve healthcare outcomes for this at-risk population. To accomplish this goal, it is important to develop an action plan. Thinking about the need you identified and the Social Determinants of Health develop an intervention action plan to address the identified need.
