Assignment Task: Care of the client with a pressure ulcer.
Client: You are assigned as the primary nurse for an 89 yr old female with severe dementia who has been bedridden due to a severe pressure ulcer on her coccyx. She is completely dependent for all ADLs, incontinent of both urine and bowel and eats and drinks poorly. The pressure ulcer started several months ago and the staff in the LTC was unable to contain it despite various nursing interventions. She has been admitted for IV hydration, wound assessment and care planning.
1. Describe and discuss the procedures or issues from the scenario that may impact the recovery and health of the medical/surgical client.
2. Describes how to best care for this client with evidence supported by literature. (Nursing interventions.)
