Behavior Change Project Assignment

Part I

Step 1: Pick 3 scholarly journals from the CSUSB library database (online) Read them. You must print and bring hard paper copy and show the Scholarly Journal Articles to the instructor.

Step 2: Pick articles that are relevant to your behavior change. Each article must be at least 3 pages.

For example if you are trying to change your sleeping habits then look for articles that focus on " sleeping habits among college students", " how to increase more sleep into your day", " benefits of sleeping" ect.

Step 3: Summarize articles. Type up summaries double spac. State the main points of the article and conclusion reached. Also include how this material will be helpful in you following through with the proposed behavior change. You should ask yourself of what value is this information to me now or later in life? (you will submit 3 separate summaries in total)

Step 4: You must include a Citation Page and in-text citations in the summary. Include Author, date published, title of article, name of journal, volume number and pages in your summaries. You may use any acceptable citation style. (citation page must be second page)

Part II

Step 1:Keep a personal Journal and write two Journals entries per week describing your progress and pitfalls as your work towards your personal goal or next step of the Transtheoretical Model discussed in Unit 3. Include barriers encountered, successes, emotional and physical reaction to changes ect.

Step 2: Type all your personal journal entries in a word document, double-spaced. You can write your journals on the same page but you have to mark "Entry 1 - Date".Each journal entry should be 4-5 sentences.

Part III

Step 1: Write an evaluation of your health behavior assessment and modification project. On Lab Unit 10, Exercise 1 (Pg 10-3)

Step2: You will find the questions to answer on Unit 10 pg 10-3.

Step3: You will type up your answers on a word document. Number your answers. Type up answers single space and be thorough when answering the questions.

Part IV

Step 1: You will complete unit 3, worksheet 3-A and Exercises 1, and 2 in lab. During lab I will sign your contract and mark each student if they completed worksheet 3-A and Exercises 1, and 2.

Task: Personal Health Behavior Change Project-A Lifelong Plan for Health

Exercise 1. Health Improvement Plan Evaluation

1. Did you follow your plan? Did your timeline work? Were journal entries helpful? Were your support people valuable? Where did your plan break down? Was your goal unreasonable? What barriers hindered your progress?

2. How do you feel about your efforts and any changes you made?

3. How will you maintain your changed behavior?

The answers to these questions should be submitted in a typed up word document and printed out, submitted a hardcopy to me.

Format your assignment according to the following formatting requirements:

1. The answer should be typed, double spaced, using Times New Roman font (size 12), with one-inch margins on all sides.

2. The response also includes a cover page containing the title of the assignment, the student's name, the course title, and the date. The cover page is not included in the required page length.

3. Also include a reference page. The Citations and references should follow APA format. The reference page is not included in the required page length.
