The Importance of Cultural Awareness and Humility in Treating Substance Use Disorders
Cultural awareness and humility are essential components of ethical and effective social work practice, especially when working with clients affected by substance use disorders (SUDs). Substance use and recovery are deeply personal experiences that are shaped by a client's cultural background, values, family systems, and belief structures. Practicing cultural humility means approaching each client with openness, a willingness to learn, and respect for the client's worldview. This aligns with the NASW Code of Ethics, which calls for competence in practice and respect for the dignity and worth of each person. Substance Abuse and Mental Health Services Administration. (2015, November).
Understanding a client's culture is critical to supporting their recovery because cultural identity often influences how clients define health, illness, help-seeking behavior, and healing. For instance, some clients may view addiction as a moral failing, while others see it as a medical or spiritual crisis. Similarly, stigma around mental health and substance use may be more pronounced in some communities, making clients less likely to seek or accept help.
Cultural background can also impact assessment and treatment by influencing the client's communication style, attitudes toward authority, and understanding of clinical language. Without cultural competence, clinicians risk misinterpreting behavior, overlooking strengths, or applying interventions that are irrelevant or even harmful.
From the nine Culturally Responsive Evaluation and Treatment Planning Steps in Chapter 3 of TIP 59, two critical steps include:
Engage Clients in a Collaborative Treatment Process: Social workers can implement this by co-creating treatment plans with clients, rather than imposing pre-set goals. For example, a clinician might explore what recovery looks like from the client's cultural lens and incorporate traditional healing practices or community supports if appropriate. Capuzzi, D., & Stauffer, M. D. (2020).
Elicit and Incorporate Cultural Strengths: Social workers should actively identify and affirm strengths rooted in culture, such as resilience, strong family ties, or spiritual practices. A practical way to do this is through culturally adapted motivational interviewing that validates the client's lived experience and supports their autonomy and agency in change.
Incorporating these steps helps ensure that treatment is not only person-centered but also culturally grounded. It empowers clients to participate meaningfully in their own healing, which can ultimately improve engagement, trust, and outcomes. Capuzzi, D., & Stauffer, M. D. (2020).

Why is an understanding of a client's culture critical to supporting the client's recovery journey?
How might a client's cultural background impact substance use disorder assessment or treatment processes?
A client's culture significantly influences their beliefs and attitudes towards addiction and recovery. It can affect a client's willingness to seek and engage in treatment. "Differences in cultural norms, practices, and beliefs have been shown to affect rates of use" (Capuzzi & Stauffer,2020, pg.366). Cultural considerations can also affect support networks and coping mechanisms for a client. Treatment plans should be specific to address underlying cultural disparities and possibly incorporate traditional healing practices. A successful therapeutic relationship does not exist without trust and rapport, which is almost impossible to build without the professional having strong cultural awareness, knowledge and skills. Cultural competence directly affects client engagement and outcomes. "Cultural competence is the ability to recognize the importance of race, ethnicity, and culture" (SAMHSA, 2015, pg. 7). Professionals should demonstrate a sense of understanding, validation and respect towards a client's culture and beliefs.
Select two of the nine Culturally Responsive Evaluation and Treatment Planning Steps listed in Chapter 3 of TIP 59: Improving Cultural Competence(Substance Abuse and Mental Health Services Administration, 2015).
Describe several practical ways that social workers could implement these best practices in their work with clients with substance use disorders.
The nine culturally responsive evaluation and treatment planning steps help professionals tailor treatment with clients around their culture background and needs. Step 2 of this process involves familiarizing clients with treatment and process evaluation. "By taking the time to acclimate clients and their families to the treatment process, counselors and other behavioral health staff members tackle one obstacle that could further impede treatment engagement and retention among racially and ethnically diverse clients" (SAMHSA, 2015, pg. 60). The professional should break complex terms down more understandable and continuously ask if the client has questions before moving forward. The environment, language and expectations surrounding treatment can seem overwhelming for new clients. This step in the process makes the initial session more comfortable and empowers the client to return. Step 6 includes selecting culturally appropriate screening and assessment tools. This may include using screening and assessment tools that are in the language appropriate and understandable for the client.
