Problem: Generate SOAP note of a 32 year old male with history of schizophrenia who came to his outpatient follow up and reported that his medications had not been effective. Patient reports that although he has been taking his medications consistently but still experience period of auditory hallucinations. He stated that the voices keep telling him to do different things and they make him feel uncomfortable for that reason. Patient presents with poor hygiene, somehow disheveled during appointment. Appeared to be internally preoccupied, distracted at times. He is currently taking risperidone 2 mg daily and seroquel 25 mg BID, then congentin 1 mg to manage his symptoms. The plain is to increase seroquel to 50 mg twice daily, keep risperidone 2 mg. Patient educated regarding side effects and medication adherence. Follow up appointment given for 2 weeks.
