Problem:

You are the PMHNP who is treating Genevieve, an 18-year-old female with a childhood history of obsessive-compulsive disorder. To date, Genevieve has been treated with sertraline at 200mg daily and fluoxetine at 80mg daily, neither of which were effective in the treatment of her OCD symptoms. She tells you that she has been in therapy since she was a young child and stopped because she did not feel it was helpful. In your evaluation today, Genevieve tells you of her difficulty in being able to drive her car, go to work, or consider moving forward in going to college because of her intrusive thoughts. She explains that she is constantly catastrophizing in her mind being crushed by another car or truck while driving on the highway so she drives on backroads everywhere she goes which takes her twice as long to get anywhere. She has been late to work so often that they have given her a warning about termination. She has been accepted to college at the local university but is required to live on campus for the first year and she is not able to even leave her house where she lives with her parents except when necessary.
