Assignment Description: You have just recently joined the educational team at the facility where you work.
The educational team provides monthly educational sessions on coding issues, reimbursement issues, and EHR software updates. One item that has been identified as missing from the training session is an evaluation of the training session.
As the newest member of the team, you have been asked to create two evaluation forms: The first evaluation form is a form that will be given to the educational session attendees to provide feedback on the session. The second evaluation form is a form that will be given to the educational session presenter(s) to provide feedback about the session.
