Problem:
I would like these paragraphs to be rewritten into different words and expanded that have be to used as a justification for an ABA report. These are the areas and skills the client is lacking and needs to be worked on.
Social Communication/Language Deficits:
To request preferred items, Michael primarily engages in hand-leading, pointing, or independently obtaining the item. He does not currently use verbal communication to mand. Michael is unable to express physical sensations, tact common objects, or indicate yes or no. In addition, he is not able to identify body parts, respond to his own name, imitate sounds or words, or consistently orient his body toward a speaker. Social Skills Deficits:
Michael is completely non-verbal, with the exception of producing random, non-functional vocalizations throughout the day. As a result, he does not yet engage in functional communication. Michael does not orient toward or initiate interactions with peers and does not attempt to approach them. He will greet others by waving only when prompted. Michael does not demonstrate an understanding of social cues or emotions and is unable to express his own. At home, he does not attend to or engage with his brother.
Play Skills Deficits:
Michael shows a preference for manipulatives, sensory manipulatives, or his tablet instead of toys. When he engages with toys, his play is not age-appropriate and he will rotate objects, line them up, or carry them while walking. He is unable to engage in imaginative play. Michael consistently refuses to participate in seatwork activities, including coloring or arts and crafts. In social contexts with peers, Michael will solely engage in isolated play and does not demonstrate and understand how to share or take turns.
Restricted, Repetitive Patterns Of Behaviors, Stereotypy:
Michael demonstrates a strong preference for routines and will engage in maladaptive behaviors they are disrupted. He has issues with certain food textures, specifically foods with soft or mushy textures such as cake, applesauce, and mashed potatoes. During car rides, if Michael sees that the car is heading home, he may engage in maladaptive behaviors. Michael also engages in a variety of stereotypic behaviors, including jumping, rocking back and forth, toe walking, hand flapping, hitting the back of his ears with his hands, and moving his arms repeatedly over his head. At home, Michael avoids entering the kitchen or basement, due to fear of navigating the way to them.
Maladaptive Behaviors:
Michael engages in loud crying behavior to gain access to attention, preferred items or activities, or to escape task demands. These episodes can typically last up to 30 minutes. He also engages in elopement from the home or classroom when opportunities arise. Elopement may occur more than 20 times per day across home and school settings, most often maintained by escape from task demands. Michael currently cannot follow multi-step directions and demonstrates significant difficulty transitioning from preferred to non-preferred items or activities. In addition, when he recognizes that he is being taken to a doctor's appointment, he may engage in both crying and elopement behaviors.
Self-Help Skills Deficits:
Michael is not yet toilet trained and cannot independently wash his hands, brush this teeth, dress or undress, put his shoes on, or use utensils during meals. He does not understand that he should not touch items that are hot or sharp. In addition, Michael does not know personal information such as his parents' phone numbers or home address. When outdoors, he does not demonstrate safety skills and will attempt to run into the street without any regard for his safety.
Parent Concerns:
Michael's mother is concerned with his inability to function age-appropriately, tact items, follow directions, and communicate his needs.
