Problem
After getting report, you begin patient care. When you enter Miss Handel's room at 0815, you note her primary IV bag of ½ NS is almost empty. You hang a new liter bag 15 minutes later. You note she has a continuous drip of Zosyn running at 6.8 ml/hr on a separate pump. She has a single peripheral 20 g IV site.
Later that morning, the patient has Vancomycin 750 mg in 200 ml ordered, to run over 90 minutes.
Two hours later, the doctor comes in and changes the primary fluid to D5.45NS.
