Problem: S.G.M It's a 27 year old male who presents for dizziness and GI problems. History of present illness a 27 year old male with history of asthma psychiatric issues and prior alcohol use seen for evaluation of recurrent episodes of nausea dizziness paresthesias syncope and significant weight loss. He reports that since early last year episodes of pins and needles sensation numbness and near fainting has occurred especially with sun exposure or prolonged standing. These episodes are unpredictable and sometimes result in actual loss of consciousness. Associated symptoms include shortness of breath yawning and sensation of impending fainting. He has experience of sensual weight loss dropping from 180 -190 lbs to a lower on a specified weight . Take eating especially in the mornings that persistent inability to work due to symptoms are noted he describes increased thirst and feeling rapidly dehydrated requiring frequent water intake headaches develop when looking up for extended periods often accompanied by business ear congestion and wooziness he monitors his blood pressure which has been low around 90 / 60 particularly at night when symptoms are present also occurs intermittently throughout the day sometimes relieved by drinking water he occasionally feels so unwell that he is confined to bed experiencing shaking and widespread paresthesias.
