Assessment - Appraisal of the Clinical Guidelines for Stroke Management 2010 related to your specific discipline scenario

Scenario

Mary Pierce, a pregnant Maori woman, has called an ambulance to her home. She is 32 weeks pregnant, with an uncomplicated healthy pregnancy up till now. She describes her symptoms to the paramedics at the scene as sudden dizziness, blindness in one eye and a 'bad' headache.

Please select one of the four discipline versions below to base your assignment on.

A. Paramedic version:

As the paramedic called to Mary's home, you have standard assessments to undertake, symptoms to observe that may indicate Mary has suffered a stroke and early management to attend to for Mary's.

Your patient management will follow standards of care and specific care issues related to Mary's symptoms at the scene.

B. Midwifery version:

Mary is transferred from the A & E Department to the antenatal ward, where you are working. She is hoping to have a healthy baby and go to full term for her pregnancy.

Your role will focus on standards of care for all antenatal women, taking into consideration Mary's Maori status, and how to monitor the wellbeing of her unborn baby.

C. Nursing Version:

You meet Mary and her newborn baby daughter in the rehabilitation unit, twelve weeks after her emergency admission to your hospital. She is an inpatient in your unit, and is now walking with assistance and has gained strength in her left arm and left leg to enable her to feed herself and walk with a stick.

Your role focuses on working within a multidisciplinary team, to support Mary's recovery, to meet her activities of daily living (ADLs), while supporting her mentally to manage her rehabilitation. Please ensure you address any specific considerations associated with Mary's Maori heritage.

Description: This assignment is based on a clinical practice scenario for students to work from to 'mimic' the five steps of the EBP process. The interdisciplinary scenario presents a clinical situation and requires students to separate relevant from irrelevant information to arrive at best practice guidelines.

Instructions:

1. From the scenario provided in the 'My Assessments' tab in LEO choose the aspect that matches your discipline.

2. Discuss the following key concepts and their interrelatedness - clinical practice variation, clinical decision making and clinical practice guidelines

3. Present a detailed evaluation of the CPG (using five criteria as noted in the marking guide).

4. Provide a detailed evaluation of how the clinical practice guideline (CPG) provides clear direction for you, with respect to the early management of the clinical problem which is the focus of your aspect of the scenario.

5. Discuss how the CPG you found helps to reduce variation in practice and any facilitators or barriers relevant to the implementation of the CPG.
