Problem: Condense in collaborative documentation. This was client's first session with clinician mandated reporting, confidentiality and limitations. Client explained therapeutic process and discuss any questions client might have following that client shared details of his childhood, current life stressors and experiences as well as past experiences that have shaped his current life such as spending time in prison. Client shared details of these experiences. Client shared reasons for seeking therapy due to anxiety depression triggers challenges but could not quite pin point how his anxiety and depression felt saying it was hard to explain.
