Problem:

When working with a client like Sara, a 22-year-old Latina-American woman who comes in reporting sadness, trouble concentrating, a few recent panic attacks, and a low appetite, the first step would be to complete a full assessment before making any kind of diagnosis. Even though her symptoms line up with things we often see in anxiety or depression, it's important not to label anything too quickly. I would start with a detailed clinical interview where I ask her about when her symptoms started, how long they have been happening, and how much they affect her daily life, I would also want to know what is going on in her personal world such as,work, family, relationships, stress, and cultural pressures,to understand the full picture. I would explore both anxiety and depression symptoms in more detail, and I would also ask about her sleep, appetite, energy levels, and any thoughts of hurting herself. Part of the first appointment would also include ruling out medical issues or anything related to substances, because those can sometimes look like mental health symptoms. Among all the assessment tools available, you will be using your eyes, your ears, your intuition, and all your senses to observe carefully what your clients are experiencing and communicating. You will attempt to decode these cues and make sense of their behavior. And you will coordinate these observations with other data at your disposal ( Kottler & Shepard, P.220). Some ideas for her treatment plan would be to focus on building rapport and making sure she feels safe and understood, especially since cultural background can affect how someone expresses emotions or views therapy. We might also work on routines for sleep, eating, and stress management. If needed, I would refer her to a medical provider for further evaluation. One psychological assessment I may use in her initial appointment would be the BDI ( Beck Depression Inventory) it consists of 21 questions about feelings and physical symptoms over the past two weeks.
