Diagnosis of adhd made by pediatrician


Problem: An 8-year-old boy, Kevin, brought in to see you for the first time by his mother. He had a diagnosis of ADHD made by his pediatrician when he was six after reports from the school that he was hyperactive, impulsive, and often off-task. Kevin has never taken medication because his father thinks he is "Just being a boy. I was just like that as a kid, and I turned out fine." The patient's mother who is the primary care giver and spends the most time with him reports that he has a great deal of irritability most days.  He has had outbursts where he hits others, damages property (i.e. kicking a hole in the wall when he struggled with a videogame) and yells in furor and anger. These outbursts are often triggered when he is told to stop playing videogames or to complete a chore. The outbursts tend to last about 10 minutes to an hour at most, and afterwards he can express what triggered them. Thoughts fly through his head, and he jumps from one idea to the next without adequately expressing the first idea. Kevin leaves messes scattered around the house and doesn't clean up after himself. When given a task to do he often claims he "forgets." His mother says she must tell him five or six times to complete a chore because he gets distracted very easily. He often challenges authority and defies rules. Homework is another trigger for outbursts when he will even sit still to attempt it.  His mother reports that Kevin's behavior is causing her a great deal of distress and conflict within the family. He doesn't have friends and is not invited to parties or playdates because other parents and children think he is "rude." You observe Kevin in your office as a curious and energetic child. He fidgets constantly, looks around a lot, touches objects on your desk, and is disrespectful to his mother. When you ask him a question, he either responds, "I don't know" or shifts the conversation to talking about videogames he is interested in. You note that he talks at you more than to you.
