Diagnosing the problem as elbow tendonitis

Problem: A 45yo male was sent by his PCP to an orthopedic surgeon's office to render an opinion for acute pain and stiffness in his right elbow. In his problem focused history, the physician noted that the man was a farmer and used his right hand and arm repeatedly, lifting heavy objects, often without assistance. The patient has no other complaints and was reported to be in otherwise excellent health. The patient described the pain as severe and unrelenting and prevented him from using his arm. The problem focused physical examination noted the man's slightly swollen right elbow, with marked pain on movement. No other problems were noted with his right upper extremity. The physician diagnosed the problem as elbow tendonitis and bursitis and recommended warm compresses and prescribed him and anti-inflammatory medication. The MDM complexity was straightforward.
