Develop a plan of care for a 24-year-old female being seen in her physician's office. Her primary complaint is "I'm just so tired all the time...and I've gained 15 pounds in the past three months."
Additional assessment reveals the following information:
- She has not been sleeping well, averaging 3-4 hours/night.
- She has missed four shifts at work over the past two weeks stating, "I just didn't feel like I could get there."
- She is beginning her 5th year of college studying communications and pre-law.
- She broke up with her boyfriend of three years "a couple of months ago."
- Her weight is 184 lb and height is 5'7".
The physician recommends she see a therapist and starts her on the following medications:
- Zoloft 50 mg po every morning
- Ambien 5 mg po every evening
Include one nursing diagnosis, two goals (one short term and one long term), interventions, and evaluation criteria.
