PURPOSE

Assessment Task is designed for you to demonstrate your knowledge about WHS issues and apply it to the scenarios. You will be required to read the hypothetical diary of a few days in a salon and then apply your knowledge of safe hygiene, health and work practices to:

- Identify the incidents which need to be addressed and discussed with the team
- Create information sheets for the staff to illustrate your knowledge of what the team require further training in
- Complete an incident and hazard report

ASSESSMENT TASK

PART A
You are required to read the following scenarios/list of incidents that happened at Dizzy's Beauty Salon during last week. Once you have read the scenarios, you are required to create three (3) hazard reports (One for each day), choosing one (1) hazard per day to report about. Please use the incident report template in e'Spa for your reports.

Dizzy's Beauty Salon
Dizzy's Beauty Salon is a busy high street salon with the following employees: Monique - Salon owner
Sarah - Salon manager/ Beauty therapist Lucy - Beauty therapist
Lola - Trainee beauty therapist
Monique - Salon owner and first aid officer

Monique, the salon owner of Dizzy's Beauty Salon is concerned about the amount of incidents and minor accidents that happened in the salon last week. She has decided to make a list of all of the incidents and address them with staff and propose a new training plan so this does not happen again.

Here's a run-down of the week:

Monday 5th March
The first appointment for the day was a facial client called Rose who was scheduled for her facial treatment with Sarah at 9am on the 5th March 2016.

1. Sarah plugged the facial steamer into the power supply in the hall way and turned it on with the steam head facing towards the open hallway while her client got changed and she made a cup of tea.

2. A delivery man from Express Orders arrived at the salon with two medium sized boxes that were marked ‘fragile/glass contents' and was instructed by Lola to place them in the busy hallway.

3. The delivery driver walked past the steamer and the steam burnt his arm, he then tripped over the facial steamer power cord and fell, smashing the boxes and content all over the floor.

4. Hot water was spilt all over the floor and the power lead was in the water still plugged in at the wall.

5. Lola attempted to pick up the broken glass with her bare hands so the delivery driver and clients wouldn't cut themselves and she cut her hand in the process.

6. Sarah came running from the kitchen after hearing the loud thud and slipped on the water and hurt her wrist.

7. The broken vaporizer/steamer was left in the hallway whilst Lola applied a wet towel to her hand to try and stop the bleeding. Monique the first aid officer came to see what had happened.

8. The delivery driver got up off the floor and brushed himself off and told the staff he was late and had to leave; he left the salon and no record of the incident was recorded.

Tuesday 6th March
Lucy was rostered on to start work at 8.30am, when she had not arrived to the salon by 9am, Monique the manager called her and Lucy told Monique she had no idea she was rostered on and she would arrive to work as quickly as she could. When she arrived at work, she was wearing plain clothes and open toed shoes. Her excuse was her uniform was dirty and her closed in shoes were at her boyfriend's house.
1. Lucy's first appointment was a half leg and an underarm wax. Lucy took her client through for the treatment and did not wash her hands or apply gloves prior to starting the waxing service.
2. Whilst performing the underarm wax, Lucy's hair fell forward and became caught in the wax that she had just applied to the client's underarms. Lucy grabbed a pair of scissors and cut her hair from the wax.
3. When Lucy removed the patch of wax from the client's underarms she applied pressure. When she moved her hand away she noticed dots of blood on her palms from pressing the client's underarms and wiped the blood on her trousers.
4. Lucy extracted an ingrown hair from her client's underarm and accidentally pricked herself with the contaminated needle. She continued with her work and threw the used sharp into the rubbish bin.
5. When the client was getting off the beauty couch after the treatment, her leg got stuck in the wax pot cord that was hanging off the bench and the wax pot fell onto Lucy's foot and badly bruised her toe. Lucy did not complete an incident report.

Thursday 8th March
Sarah arrived to work and was switching on the lights in each room and re-stocking items ready for the day ahead when she noticed:
1. The wax pots in the waxing room kept switching themselves off when she would turn them on. They were also making a buzzing noise.
2. Sarah noticed that the power board had six other electrical items plugged into it.
3. As Sarah was touching the power board, she noticed one of the cords was frayed and tried to remove it and got an electric shock.

4. As she was shocked, she accidentally knocked over a large professional size bottle of acetone that she was using to re-fill the acetone bottles. The acetone went all over the bench and power board and the floors.
5. Lola heard Sarah's cry for help and entered the treatment room and slipped on the spilt acetone and got some splashes in her eyes. She began to rub her eyes and the stinging got worse.
6. Monique the salon owner came rushing in to find the two employees in distress. Lola explained what happened and that she used water and tissues to clean her eyes but the stinging wouldn't go away; her eye was very red.
7. Monique searched for an SDS sheet but couldn't find one anywhere in the salon. She googled the SDS to find out what to do.
8. Sarah in the meantime was in shock and felt dizzy but continued on with setting up the rooms.
9. The buzzing wax pot with acetone on the power board was switched off at the wall and the wax pot left in the room.

Part B
In the template below, you are required to create a meeting agenda outlining each incident that happened on each day and address the following points to talk about in your upcoming workplace health and safety meeting with your fellow staff. You are required to use the hierarchy of risk control to help with your response on the correct procedure for each.

steamer should have been plugged in close the the work area to prevent hazards

Stream head facing the door way

The steamer should have been facing a wall or the floor when it is turned on

Hot water spilt over the power board

Turn off the power board immediately before unplugging equipment and the power board. Pick the power board off the floor and dry it. It may need to be tagged as faulty

How should this incident have been correctly handled and reported?

Monday

Picking up broken glass with bare hands

Isolate the area, put of PPE especially gloves and clean up the glass using a dustpan and broom. Dispose safely in the waste bin

Running in the salon

Could be avoided by only walking in the salon

Broken steamer left in the hallway

The steamer should have been placed to the side and tagged as faulty

No incident form was filed

All incidents no matter how small needs to be filled out

Delivery driver told to place parcels in busy hallway

If a area is busy, the parcels should be placed some where else that is not busy

Tuesday

Did not wash hands or apply gloves

Hands should be washed before and after treatments and gloves need to be worn in all skin penetrating treatments like waxing. This should be reported to the manager or supervisor

 

 

 

Could have been avoided if hair was tied back. This should have been reported to the supervisor and a incident report form filled out. The wax pot is now contaminated and should be discarded

 

 

 

iIf Lucy had of followed personal hygiene requirements and tied hair back this would not have occured. The wax pot needs to be disposed of appropriately as it has been contaminated. Inform the supervisor and the incident report form filed out

Turn the pots off and unplug them, they need to be assessed, tag as faulty inform supervisor

Only plug in the appliances as they are being used and unplug after using them, to prevent overloaded. Inform supervisor

Thursday

Cord frayed

Cords need to be checked before being plugged in, put equipment to the side and mark as faulty. inform supervisor

Knocked over acetone

The acetone should have its lid on as soon as it was sat back on the bench an not placed close to the edge of the bench The supervisor should be informed and a incident report form filled out.

Slipped on acetone got in eye

The room should have been assessed before entering so the spill could have been walked around. The spill should have been cleaned up as soon as it happened.

The supervisor should be informed and a incident report form should be filled out.

No SDS form in the salon

All forms should be at the front desk and easy to be found. If the forms are run out they should be replaced, and the forms can be found on the SDS website

The supervisor should be informed

Sarah still setting up and wax pot left in room

Sarah should be sitting down with ice cold water and if necessary send to doctor. Wax pot should be labeled as out of order and replaced with a working wax pot

The supervisor should be informed and a incident report form filled out.

Acetone all over power boards

Immediately turn of the power and unplug power boards, remove cords and dried before using the board again. The power board may need to be tagged as faulty. Inform supervisor and a incident report form may need to be filled out.

PART C
Now everyone is informed of the above incidents and what the correct procedure for each incident is, you are required to help prepare staff training leaflets that will be distributed to the employees at the upcoming workplace health and safety consultation meeting:

- How to Avoid Cross Infection -List the three (3) means of preventing cross- infection.
- List the three (3) different forms of disinfection and give one example of where you would use each in the salon.
- Electrical safety in the salon - Create a guideline for electrical safety in the salon (Do's and don'ts of):
Faulty equipment that short circuits
What causes an overloaded circuit and how to prevent this? Electrical safety devices that can be used to prevent electrical faults Briefly explain the effects of electrocution and electrical shock.
- Procedure for blood soiled linen
- Importance of completing an incident report

PART D
You are required to help educate the other staff about how to respond to an emergency evacuation due to a fire in the salon. Using your learning content, please list the steps you would take to follow organisational emergency procedures? Please include:
- Fire and emergency procedures

Alert everyone in the salon and the building warden, stay calm and exit out of the closest door and ensure everyone is following, heading anyone struggling to leave, if possible and safe to do so take hand held personal items like hand bags with you.
Head straight to the assembly point and stay there until told by the building warden or emergency services. Contact emergency services if they haven't been notified already
If trained and safe to do so use of fire equipment - attempt to extinguish the fire. Choose the correct fire extinguisher. Do not use water or foam on an electrical fire.

Follow the instructions for the building and area from managers and supervisors When the evacuation alarm is heard, prepare to leave the building immediately ensuring confidential materials and valuables and if possible collect personal belongings and turn off all electrical equipment.

leave the building as instructed by supervisor or manager, by the nearest and safest exit route shutting all doors on the way out.
Take hand held belonging is possible like handbags, don't return to collect belongings.
Help any person having difficulties leaving the building.
Walk quickly and calmly to the designated assembly area for the building or advised by the manager or the fire and emergency services personnel.

Stay in the assembly area in groups and do not reenter the building until instructed to leave by manager or fire and emergency personnel.
