Do a web search of your state's department of health website. Look specifically for how your state defines "social determinants of health". What are high risk populations, and what are strategies and/or interventions your state has implemented to address these issues? Here are websites that can be used for those living in Wisconsin or Minnesota.

1. There are many factors identified in the documentary: Escape Fire, that drive up health care costs. Choose one factor to discuss, and using an example from the film or your own personal practice, discuss the impact it has on the cost of health care. Additionally, after researching your state's department of health's website, discuss what your state is, or should be doing to address the issue.

As a health care provider that has a vested interest in caring for the patients in your community, and after reviewing information on your state's department of health website, discuss several of your state's strengths and opportunities for improvement aimed at health inequity and disparity. Additionally, discuss and post the link to one strategy, program, and/or initiative your state has initiated to address inequity and/or disparity. This may be a link to an article, video, bulletin, or news release.
