Assignment task: Mr McDonald is 45 years old and was diagnosed with a Stroke (PACS) 18 months ago. Following rehabilitation, he now mobilises with 1 stick and uses an AFO on his left side. He can walk short distances indoors independently but requires a stick for outdoors and is limited in the distance he can walk. He requires assistance to apply his splint and shoes and has a left hemianopia, impaired concentration and short term memory issues. He lives in a house with stairs with his wife and 2 children (12 & 8 years old), and his wife undertakes most of the domestic tasks. He previously worked as a Salesman but was unable to return to this and took medical retirement. He is keen to consider volunteering or re-training to return to some employment. He also previously enjoyed playing golf, football and socialising with his family and friends.

This assessment aims to provide an opportunity for you to demonstrate achievement of the following learning outcomes:

1 Critically analyse the inter-relationship of the aetiology, pathophysiology, clinical features, course and prognosis for common neurological conditions

2 Critically discuss the impact of sensory-motor, cognitive, communication, behavioural problems and psychosocial needs on the quality of life of patients and their carers as part of the clinical reasoning process and reflect on the implications for physiotherapy management

3 Critically apply knowledge and understanding of the features of normal movement, conduct and record a neurological assessment using the International Classification of Functioning, Disability & Health .

4. Critically discuss what you would wish to assess in this person and why, taking into consideration factors such as pathophysiology and clinical features which may be common in the identified case study

5. Critically reason through your short and long term treatment options, using evidence to support your suggestions, and identify short and long term goals appropriate to this. You should ensure that you take into consideration the impact of sensory-motor, cognitive, communication, behavioural problems and psychosocial needs on the quality of life of patients and their carers.
