Question 1:
Design a 10-question questionnaire based on your proposed research problem. The questionnaire must contain an introduction that explains the purpose, an approximate time frame for completion, and a statement on confidentiality/anonymity. There should be at most two open-ended questions. Please review the guidelines located in Chapter 15. You may also refer to Appendix 15A for wording on the most common research topics.
Your response should be at least 200 words in length, unless otherwise specified. You are required to use at least your textbook as source material for your response. All sources used, including the textbook, must be referenced; paraphrased and quoted material must have accompanying citations

Evaluate each question that you included in the questionnaire. Explain how the question will help meet the objectives of the research. Please be specific and provide justification.
