Identify either a product or system on which to perform a PHL (Preliminary Hazard List) OR a job or task on which to perform a JHA (Job Hazard Analysis). You should create your own PHL form using the examples in the class notes for Formal Safety Analyses (spreadsheets usually work fine). If you perform a basic PHL, your form must include the three categories discussed in the class notes (four if you add in RAC/probability). A JHA needs, at a minimum, task description, hazards and preventative measures. A level of risk assessment may also be added. (limit this two 2 pages max.—be detailed and specific in your 2 pages, but you can stop at 2 pages even if you are only partially complete).
