You need simple php form code with handling form. You need the following features in the form in the following order
1 picture on left side with checkbox in middle and price on right ($10)
1 picture on left side with checkbox in middle and price on right ($10)
1 picture on left side with checkbox in middle and price on right ($10)
1 textbox for name
1 textbox for address
date selector
1 text box for creditcrd
SUBMIT button
If no name or address is entered ask to go back and fill
Handling form should calculate total plus 9% tax plus shipping $5 and result should have all information that was inputted from the form on one page and showing shipping fee and tax and then total.
