Choose a process, system, design or facility that you deal with in your field of study that can be the subject of a risk analysis. Provide a description of the item you have chosen. Use one or more of the steps discussed in class to identify an initiating event for a risk that can arise from the item of your choice, through to a possible onsite or offsite consequence. Your response should not be more than 2 pages long.

1. You are working in a company in which senior management has decided that expansion of the current production plant is required. Building a new plant with new equipment and new control systems is too expensive. The current plant, when expanded, will contain a mix of new equipment and old equipment, new control systems and old control systems, and new people mixed with the older people that have been working for the company for years. As an engineer, and the only individual who has ever taken a course in risk assessment from the University of Regina, you are assigned the task of ensuring that everything will work together (and safely) in the expanded plant. You decide that hazards identification is the first step that must be accomplished. You can’t use all the methods covered in class, so you will have to pick one or two of the methods. What method (methods) will you choose, and explain why you would choose it (or them) and how it/they will be applied.

2. Explain what is meant by the following term and what significance it has in terms of risk assessment and risk management (use your own words):

Incommensurability of outcomes
