Dale Evans is an 86-year-old patient that has been assigned to your team. Evans was admitted to the hospital yesterday with dehydration and cellulitis of the right lower leg. Medical orders include IV therapy/ fluid replacement, IV antibiotics, and dressing changes to the right lower leg. In the RN's hand off report, the patient's right leg pain is increased, now with a score of 7 on a scale of 0-10. Vital signs at 0600 include temperature of 100.4 degrees F, heart rate at 98 beats per minute, respiratory rate 22 breaths per minute, and a BP of 148/70 mmHg.

You are working with an Assistive Personnel (AP) and an LPN/LVN. You will be delegating the tasks of obtaining vital signs every 2 hours instead of the routine every 4-hour schedule, changing the dressing to the right leg, monitoring intake and output every 2 hours, administering the IV antibiotic, providing morning care, and repositioning the patient every 2 hours.

1) Develop an SBAR which describes Dale Evans' condition to the other members of your nursing team.
